Social Work
Practical support, advocacy and guidance through complex circumstances
Choice Action provides personalised social work support when life circumstances or different service systems become difficult to manage.
We help you understand your options, address barriers, connect with appropriate supports and take practical steps towards greater stability, independence and informed choice.
How we can help
Support may include:
- Social work and functional assessments
- Reports and recommendations about your support needs
- Advocacy to help ensure your voice, choices and support needs are understood
- Assistance to understand and access NDIS, health, housing and justice services
- Referrals and connections to appropriate services and support networks
- Preparation for meetings, assessments, court processes and service transitions
- Support to develop self advocacy, decision making and problem solving skills
- Liaising with services on your behalf and with your consent
Housing Support
Choice Action can help you explore housing options, complete applications, gather supporting information, address tenancy difficulties and connect with housing and homelessness services. Choice Action does not provide accommodation.
Support During and After Custody
Where access is permitted, Choice Action can support people while they are in custody and assist with planning for their transition back into the community. This may include identifying support needs and helping establish suitable disability, housing, health, income, education and employment supports.
Work and Development Orders
Emma supports eligible people with Work and Development Orders (WDOs), including establishing the order, coordinating approved activities and monitoring progress to help reduce eligible NSW fine debt.
Start with a conversation
Every person’s circumstances are different. We will listen to what is happening, help identify the areas requiring attention and explain how Choice Action may be able to assist.
