Person-centred disability support activity

Choice Action provides person-centred disability support for people with disability. Our support is shaped around what you want to do, the skills you want to build and the goals that matter most to you.

We work alongside you and, where appropriate, your support networks to help you make informed choices and develop practical supports that fit into your everyday life.

Our team includes staff with over 10 years of experience in the disability sector across Sydney and the Hunter.

How we can help

Support may include:

  • Social and Community Participation: Support to take part in social, recreational, cultural and community activities that interest you. This may include planning and attending activities, trying new experiences, getting comfortable in new environments, meeting new people, building relationships, and growing your confidence and independence in the community.
  • Skills Development: Support to learn and practise everyday skills at a pace that suits you. Practical skill-building may include developing routines, planning your time, budgeting, shopping, preparing meals, using public transport and managing daily responsibilities. Social and communication skills may include starting and maintaining conversations, understanding social cues, building relationships, managing disagreements and increasing confidence in social or group settings.
  • Finding and Keeping a Job, Education and Volunteering: Support to find and keep a job by identifying your strengths, interests and goals, and exploring suitable employment, education, training or volunteering opportunities. This may include support to prepare resumes and job applications, practise interview skills, develop workplace communication and routines, build confidence, and support you to start, transition into, and maintain meaningful employment or other suitable roles.
